Passport Requirements

Evidence of Citizenship:

  • State certified birth certificate with an original certified seal from the state in which you were born (hospital birth records NOT accepted)
    OR
  • Previous U.S. Passport, Original Naturalization or Citizenship Certificate
    (Children 0–13 years with previous passport, must also submit birth certificate)

Proof of Identity:

  • State drivers license - expired forms of ID are not accepted. If applicant is under age 16, parent's id must be used
    OR
  • Valid U.S. Passport OR Military ID OR State ID card only if accompanied by other forms of valid picture id (see passport agent for more information)

Photographs:

Archer Photography, Kinko's, Office Depot, Rite-Aid, Wal-Mart, and other photo studios can take your passport photo

Fees:

  • $75.00 personal check (eff. 2/01/2008) or money order to Passport Services for applicants 16 and older
    OR $60.00 personal check (eff. 2/01/2008) or money order to Passport Services for applicants 15 and younger
    AND
  • $25.00 (eff. 2/01/2008) personal check, money order or cash to Latah County

Passport will be mailed to applicant's address:

  • Within 6 weeks maximum
  • Within 3 weeks maximum w/ $60 expedite fee
  • Within 2 weeks maximum w/ $60 expedite fee and itinerary

Passport Application Procedure:

  • Age 0 - 15 (eff. 2/01/2008), both parents must appear w/ photo ID; child must be present
  • Age 16 & 17, for security reasons, parental consent may be requested

*** EFFECTIVE FEBRUARY 1, 2008, CHILDREN AGE 15 AND UNDER MUST APPEAR WITH BOTH PARENTS AT TIME OF APPLICATION ***
